Stage 1 – Your Contact Information (for us)
The first page of the form we want to know how to contact you about the trademark. This is not what will be submitted to the USPTO (US Patent and Trademark Office) but instead what we will use for admin purposes.

Stage 2 – Information To Submit to USPTO
Now, the second stage of the form will ask us for the details you would like us to submit to the trademark office.
Filing as individual or company?
Name of the owner – the company or individual’s name (in English)
Email for trademark – this is the email that will be submitted to USPTO and used for USPTO correspondence. You’ll need to get this email for Brand registry confirmation from Amazon FYI. If you would like, we can be the one to receive the emails and send you alerts, please speak to your account representative.
Phone number – optional
Address – what address do you want to show on the trademark
Stage 3 – Information on the Trademark
After we have collected the information of who the formal owner of the trademark will be, we need to next define the trademark and the use case.
Text or logo mark? We suggest text.
The trademark (text) we suggest you to do a text-only trademark most cases. What are the exact words you would like to use?
What category – what category is this trademark in? If you have a similar mark you’d like to base this off of, put that in the notes (see below)
Photo or Website Showing it in trade / in use. This is not like domains and you can register an idea. We need to show the trademark office this trademark is being used in commerce. The best is to show a shopping cart page (in USD), and packaging of the product to show that this in fact being sold in “real life”
